I had this issue and it was a combination of things.
First I was working with the flat-faced forward set trigger and forward set sear kit. I installed the competition spring set and not the springs that came with the kit. That was my first mistake as I got the dreaded sear flutter issue that would not reset the gun and I had to manually rack the slide to get the next shot to fire.
Next was the trigger bar loop which needed to be adjusted to get full function. I slowly opened the loop until I got reliable motion on the sear.
Well, that created a reset issue that was causing the loop to hit the side of the sear and not reset properly. Readjust the trigger bar to where it was a few thousandths under the sear and would reliably cycle the striker.
Then I was getting an issue with the tang on the trigger bar rubbing the sear block enough to where I was not getting a clean reset. The issue there was a casting mark inside the sear block where the tang sits so I used a curved riffler file to "lightly" remove this casting mark and then polished the area of the sear block and the trigger bar tang with 1000 grit sandpaper.
Now my trigger works flawlessly with max power 40 cal loads and is right at the 3.3 lb mark with a very short 1911ish trigger stroke and feel.
If you install the flat faced forward set kit, watch the videos and do NOT install the competition springs, use the ones with the kit.
Wish I had listened to Randy Lee at Apex in the first place, but I really learned how the M&P trigger and disconnect works.
